After experiencing a life-altering diagnosis, individuals often have difficulty re-establishing their occupational identity (Scaffa, Reitz, & Pizzi, 2010). When individuals are discharged from the rehabilitation hospital, they no longer have the constant support of healthcare professionals, so they often struggle with community reintegration (Walsh et al., 2015). The purpose of this doctoral capstone experience was to determine if an occupation-based wellness program may benefit the community surrounding Community Rehabilitation Hospital in Indianapolis to promote community reintegration, prevent social isolation, help community members re-establish their occupational identity, and improve overall well-being (Devine & Usher, 2015; Hand et al., 2017; Scaffa et al., 2010; Walsh et al., 2015). Findings from the needs assessment conducted illustrate that establishment of this program fits the needs of individuals post-rehabilitation. Through the lens of an occupational therapy (OT) perspective, theory was used to guide the development of this program. The process of program development was measured by the goal attainment scale (GAS). Four goals were met through initiation and development of the program. An implementation plan for this program was created including educational sessions and occupation-based wellness events. To initiate this program, an occupation-based wellness event, an adapted yoga session, was planned and accomplished. Support group educational sessions were lead and occupation-based wellness activities were planned for the future. It is recommended that the occupation-based wellness program continues by planning events for support groups and then expanding to its own entity by collaboration between the program lead, staff, and executive board at the rehabilitation hospital.
